I understand that by signing this form, I am voluntarily giving permission for my minor child to participate in Post Secondary Career Exploration & Training with Cosmetology being held on February 26, 2026, at Douglas J. Institute in Knoxville, TN. Further, I attest that I am the parent/legal guardian of the child listed below and have the authority to execute this document.

I understand and acknowledge that this activity is voluntary and is not a mandatory part of any Greeneville City Schools program.

If my child has any known allergies or medical conditions, I agree to communicate this information before the trip.

I confirm that my child has no known medical conditions that may pose a risk to the health and safety of others participating in the activity. I agree to advise the instructor/supervisor of this activity, named above, in writing, of any medical, physical, or health condition that may be affected or jeopardized by participating in the activity.

In the event of an accident or illness, I agree that GCTA and Greeneville City Schools and its personnel are in no way legally or financially responsible or liable for any claim arising from any consent given in good faith in connection with diagnosis or advised treatment.
